What Is Shoulder Dystocia and What Causes It?

When a mother is giving birth, shoulder dystocia can result if the infant comes through the birth canal headfirst and the anterior shoulder of the infant cannot pass below the mother’s pelvic bone. When this happens, the baby requires a significant amount of adjustment to pass through the birth canal. Because the baby’s shoulder becomes impacted inside the mother’s body, shoulder dystocia is a risk for the mother and the baby.

Shoulder dystocia occurs in about one percent of vaginal births. If a mother is past her due date, the risk of shoulder dystocia increases. Depending on the severity of the injury and whether there was a prolonged lack of oxygen during or after birth, shoulder dystocia can result in permanent brain damage or, in extreme cases, death.

Although it is rare, shoulder dystocia usually happens when the mother’s pelvis is small and the baby is large. This condition is known as cephalopelvic disproportion. In some cases, if the mother has gestational diabetes, or if she is carrying multiple babies, shoulder dystocia injuries are more likely to occur.
